Fauquier Health Home Care Services will celebrate its 25th anniversary in August. Jerry Hoke has particular cause to be proud; he was there on day one. I can’t imagine doing anything else or working for any other organization. Many of my patients have something big in life they can’t do — like walk across the floor, get out of bed or feed the cat. When suddenly they can do that again, there is such happiness on their faces. It’s the most gratifying moment in home care.

This is an annual list of the most successful home health care providers in the U.S. Fauquier Health’s Home Care Services ranked in the top 25 percent of providers in the country for quality outcomes, quality improvement and financial performance measures. Only eight hospital-based agencies in Virginia made the list; only 25 total – including independent for-profit agencies – were chosen in the entire commonwealth.

Our skilled Home Care Services team—consisting of registered nurses, licensed therapists, certified home health aides and medical social workers—is committed to helping patients enjoy life and remain as independent as possible. In addition to providing clinical services, the staff also answers questions, listens to concerns, and educates patients and their families about the proper ways to manage their healthcare needs.
Home Care Services can help you:
Recover from an illness or surgery
Learn how to manage pain and conditions such as diabetes, heart disease, stroke, injuries, and joint/mobility problems
Receive intravenous infusion or tube feeding therapy at home
Communicate with clinicians and physicians to ensure optimal care
We offer comprehensive home care services:
Nurses provide wound care, collect lab work, administer medications, monitor for complications and teach others to care for the patient
Physical, occupational and speech therapists provide rehabilitation services such as strengthening exercises, therapy for balance and walking, home safety, daily living activities and help with assistive device use
Home health aides provide assistance to qualifying patients when they are unable to perform self-care activities
Medical social workers provide emotional and/or financial counseling, locate community resources and help plan for health care needs
To qualify or our home care services, a patient must:
Be a resident within our service area in Northern Virginia, including Fauquier, Rappahannock Counties and parts of Loudoun, Culpeper and Prince William Counties (Gainesville, Haymarket, Bristow and Nokesville.)
Be under the medical supervision of a physician
Require intermittent, professional skilled health care services in the home setting
In most cases, be homebound or unable to leave home without considerable effort and assistance
